VS02 - Explant of an Embolized Left Atrial Appendage Occlusion Device

Summary: This case highlights an open retroperitoneal approach to explant an embolized left atrial appendage occlusion device in the visceral aorta. Endovascular retrieval of the device was deemed to be an inferior alternative given the significant mural thrombus that extended towards the superior mesenteric artery and celiac artery from the device, as demonstrated on computed tomography angiogram, and the associated risk of embolizing the visceral vessels or the lower extremities. After explantation of the device and thrombectomy of the visceral aorta and vessels, the aorta was closed with a bovine pericardium patch and the patient had excellent flow in the visceral vessels on Doppler interrogation as well as palpable femoral and dorsalis pedis pulses bilaterally.
